Thrombocytopenia — low platelet count: GP workup and red flags

Thrombocytopenia (platelets below 150 × 10⁹/L) is graded mild (100–149), moderate (50–99), severe (20–49), and very severe (below 20). First step: repeat in a citrate tube and inspect the blood film to exclude platelet clumping artefact.

The differential spans immune thrombocytopenia (ITP), drug causes (heparin → HIT), life-threatening thrombotic microangiopathy (TTP, DIC, HELLP), bone marrow failure, infection (HIV, HCV, dengue), and pregnancy conditions.

Urgent haematology: platelets below 20, suspected TTP or HIT, schistocytes on blood film, blasts, or haemolysis with renal failure.

What a low platelet count means

Finding a low platelet count on a routine full blood count (FBC) is a common scenario in Australian general practice. Thrombocytopenia is defined as a platelet count below 150 × 10⁹/L. The severity classification guides urgency:

The differential diagnosis spans the entirely benign (artefactual EDTA-induced clumping) to the immediately life-threatening (thrombotic thrombocytopenic purpura, acute leukaemia). Systematic triage determines which clinical pathway applies.

The RCPA Manual and Therapeutic Guidelines — Haematology emphasise that thrombocytopenia should never be assumed benign without a methodical first-step assessment.

A. Core clinical — the AU general-practice framework

First-step triage: five questions

1. Is the count real?

EDTA (the anticoagulant in standard purple-top blood tubes) causes platelet clumping in approximately 1 in 1,000 samples, producing a falsely low count on the automated analyser. Repeat the FBC in a citrate (blue-top) tube. Call the haematology lab and request a blood film review — platelet clumps are visible under the microscope. If the repeat count normalises, pseudothrombocytopenia is confirmed and no further workup is needed.

2. Is the patient bleeding?

Assess bleeding severity using the ISTH Bleeding Score. Sites to ask about: petechiae, purpura, bruising beyond the threshold of minor trauma, bleeding from gums, heavy menstrual bleeding, blood in urine or stool, epistaxis, haemoptysis. Ask about current anticoagulant or antiplatelet medication — these amplify bleeding risk at any platelet count.

3. Could this be a thrombotic microangiopathy emergency?

The blood film is the most important test in this context. Schistocytes (red cell fragments) on the blood film, combined with low platelets plus haemolysis (raised LDH, low haptoglobin, anaemia), is a haematological emergency — possible TTP, HUS, HELLP, or DIC. Same-day haematology assessment is required. Do not wait for ADAMTS13 results to initiate transfer.

4. Is the patient on heparin?

Any heparin use — including low-molecular-weight heparin, unfractionated heparin flushes, or heparin within the prior 100 days — combined with a platelet count drop of ≥50% from baseline raises HIT (heparin-induced thrombocytopenia) as a diagnosis. The 4T scoring system (Timing of platelet fall relative to heparin, degree of Thrombocytopenia, Thrombosis or other sequelae, alternative aetiologies) stratifies pre-test probability. High-probability 4T score: stop heparin immediately and switch to a non-heparin anticoagulant while awaiting anti-PF4-heparin antibody testing. Do not wait for the antibody result before stopping heparin in a high-probability case.

5. Is the patient pregnant?

Always perform a urine or serum β-hCG in any woman of reproductive age with thrombocytopenia. Gestational thrombocytopenia (the most common pregnancy-related cause), pre-eclampsia with HELLP syndrome, and acute fatty liver of pregnancy all present with low platelets and require different management pathways.

History to take

  • Drug history — heparin (doses, dates), valproate, quinine (including in tonic water), vancomycin, sulfonamides, beta-lactams, NSAIDs, supplements (fish oil, ginkgo, garlic), recent chemotherapy or immunotherapy (immune checkpoint inhibitors), COVID vaccination (VITT — adenoviral vector vaccines, relevant for historical cases)
  • Systemic symptoms — fever, weight loss, night sweats, lymphadenopathy, fatigue (haematological malignancy)
  • Autoimmune history — lupus, Sjögren’s syndrome, antiphospholipid syndrome, thyroid disease
  • Infection history — HIV risk, HCV exposure, recent viral illness, overseas travel (dengue in north Queensland or tropics, malaria)
  • Alcohol intake — liver disease and hypersplenism
  • Family history — inherited platelet disorders

Examination

  • Vital signs — fever, hypotension, tachycardia (sepsis, TMA)
  • Skin — petechiae, purpura, wet purpura in mouth (very severe thrombocytopenia), jaundice
  • Lymph nodes — lymphadenopathy raises lymphoma or infection
  • Spleen — splenomegaly suggests liver disease, haematological malignancy, or portal hypertension
  • Neurological — new confusion or focal deficit with low platelets and haemolysis points to TTP

B. What the blood film tells you — urgent red flags

The RCPA Manual defines blood film morphology as the most information-rich single investigation in the workup of thrombocytopenia. Phone the laboratory to request a film if one has not been reported.

C. Common causes and their management

Immune thrombocytopenia (ITP)

ITP is isolated thrombocytopenia without identifiable cause (primary) or secondary to SLE, HIV, HCV, HBV, H. pylori, CLL, or lymphoma. The FBC otherwise shows no other abnormality and the blood film shows no fragments or blasts.

Per ASH 2019 guidelines (Neunert et al.) and Provan et al. 2019 international consensus:

  • Platelets ≥30 × 10⁹/L without bleeding — observation is appropriate in most patients; treatment is not automatically required
  • First-line treatment when needed — prednisolone 1 mg/kg/day for 2–4 weeks (PBS general schedule), or dexamethasone 40 mg/day for 4 days
  • When rapid response is needed — intravenous immunoglobulin (IVIG via National Blood Authority criteria) raises platelets within 24–48 hours
  • Second-line — thrombopoietin receptor agonists (eltrombopag [Revolade] or romiplostim [Nplate], PBS Authority Required, specialist-prescribed), rituximab, splenectomy

Secondary ITP: investigate for and treat the underlying cause — H. pylori eradication, hepatitis C treatment, HIV management.

Heparin-induced thrombocytopenia (HIT)

Per ASH 2018 guidelines (Cuker et al.) and Greinacher NEJM 2015:

  • Platelet drop ≥50% from baseline 5–10 days after starting heparin (or earlier on re-exposure within 100 days)
  • HIT is prothrombotic — patients develop clots (PE, DVT, arterial thrombosis), not bleeding
  • Intermediate or high 4T score: stop heparin immediately, start alternative anticoagulant (fondaparinux off-label, argatroban, or danaparoid via specialist)
  • Never give platelet transfusion in HIT — it fuels the prothrombotic state
  • HIT antibody (anti-PF4-heparin) testing confirms diagnosis — but do not delay stopping heparin awaiting results in high-probability cases

Thrombotic microangiopathy — TTP

TTP causes microangiopathic haemolytic anaemia (MAHA) plus thrombocytopenia. The classic triad on investigation is: schistocytes on film, raised LDH, low haptoglobin. ADAMTS13 activity below 10% confirms TTP.

Per ISTH 2020 guidelines (Zheng et al.) and HSANZ:

  • Mortality untreated approaches 90%; treated with urgent plasma exchange survival exceeds 80%
  • Transfer to a tertiary centre immediately — plasma exchange must not be delayed
  • Caplacizumab adjunct (via SAS or specialist approval) reduces time to platelet recovery
  • Avoid platelet transfusion in TTP

Drug-induced thrombocytopenia

Review all medications, herbal supplements, and recent vaccinations. The commonest culprits: valproate, quinine (including in tonic water — ask specifically), vancomycin, sulfonamides, beta-lactams, GPIIb/IIIa inhibitors, immune checkpoint inhibitors. Stop the causative agent. Recovery is usually within 5–10 days.

Bone marrow failure

Bicytopenia or pancytopenia plus thrombocytopenia raises concern for aplastic anaemia, myelodysplastic syndrome, acute leukaemia, lymphoma, or metastatic malignancy infiltrating the marrow. Blasts on the blood film are a haematological emergency. Any pancytopenia requires urgent bone marrow examination via haematology.

B12 and folate deficiency causes megaloblastic pancytopenia — macrocytic anaemia plus mild thrombocytopenia and leucopenia, with hypersegmented neutrophils and oval macrocytes on film. Corrects with B12 or folate replacement (see b12-folate-deficiency).

HIV can cause thrombocytopenia at any disease stage and may be the presenting feature. HCV, HBV, EBV, and CMV are other causes. In travellers returning from tropical regions, dengue (including north Queensland) and malaria are important. Offer HIV, HBV, and HCV serology in any patient with unexplained thrombocytopenia.

ADAMTS13 activity and anti-PF4-heparin antibody testing are performed at tertiary haematology or immunohaematology laboratories, typically ordered by and billed via the specialist.

  • Eltrombopag (Revolade) — PBS Authority Required (Streamlined); chronic ITP refractory to first-line; specialist-prescribed

  • IVIG (intravenous immunoglobulin) — National Blood Authority criteria; restricted indications; approved for acute severe ITP or peri-procedure
  • Rituximab — often via hospital or private pathway for ITP; PBS Authority for other indications

  • Eculizumab (Soliris) — PBS Authority for atypical HUS (complement-mediated)

Referral pathways

Per HSANZ and Therapeutic Guidelines:

  • Same-day haematology or ED — platelets below 20 × 10⁹/L with or without bleeding; suspected TTP, HIT, acute leukaemia, aplastic anaemia; pregnant patient with falling platelets, haemolysis, or hypertension
  • Urgent public haematology outpatient (2–4 weeks) — persistent unexplained thrombocytopenia, suspected MDS or lymphoma, bicytopenia
  • Obstetric haematology — pregnancy-related thrombocytopenia, ITP in pregnancy, peripartum platelet planning
  • National Blood Authority — IVIG approvals

E. Special populations

Pregnancy

Gestational thrombocytopenia is the most common cause of thrombocytopenia in pregnancy — typically mild (platelets usually ≥70 × 10⁹/L), third-trimester onset, normal LFTs and renal function, resolves post-partum. No treatment is needed.

Pre-eclampsia with HELLP (Haemolysis, Elevated Liver enzymes, Low Platelets) is an obstetric emergency. TTP can present or worsen during pregnancy, often peri-partum. Antiphospholipid syndrome causes recurrent thrombocytopenia alongside miscarriage or thrombosis. Any pregnant patient with platelets falling below 100 × 10⁹/L or with haemolysis or abnormal liver function requires urgent obstetric or haematology review.

Older adults

Unexplained thrombocytopenia in adults over 60 warrants a lower threshold for bone marrow examination — the yield for MDS and lymphoma is higher than in younger adults. New thrombocytopenia in this age group should not be assumed to be ITP without marrow assessment.

Aboriginal and Torres Strait Islander peoples

Higher prevalence of some inherited haemoglobin conditions (thalassaemia trait, haemoglobin variants) that may coexist with acquired thrombocytopenia. Culturally safe specialist referral through Aboriginal Medical Services partnerships is important. Screen for HBV and HCV given higher community prevalence.

Children

Acute immune thrombocytopenia in children aged 2–10 following viral illness is typically self-limiting — 80% resolve within three months without treatment. Observation is usually appropriate. Chronic ITP in children may require specialist paediatric haematology input.

When to escalate

Refer same-day to haematology or emergency department if:

  • Platelet count below 20 × 10⁹/L — or below 50 × 10⁹/L with active bleeding
  • Schistocytes on blood film with haemolysis — suspect TTP or HUS; initiate transfer for plasma exchange without delay
  • Blasts on blood film — acute leukaemia
  • High-probability HIT — stop heparin, start alternative anticoagulant, urgent haematology
  • Fever plus thrombocytopenia — sepsis plus DIC pathway; suspect severe infection
  • New neurological symptoms or renal failure with thrombocytopenia and haemolysis — TTP until proven otherwise
  • Pregnant patient with falling platelets plus hypertension or haemolysis — obstetric emergency

Refer urgently (within 1–2 weeks) for: persistent unexplained thrombocytopenia after initial workup, bicytopenia or pancytopenia, lymphadenopathy or splenomegaly, constitutional symptoms (weight loss, night sweats, fatigue).

Frequently asked questions

  • What does it mean to have a low platelet count?

    Platelets are the blood cells that help form clots to stop bleeding. A low platelet count (thrombocytopenia) means there are fewer platelets available to do this job. Mildly low counts (100–149 × 10⁹/L) often cause no symptoms and are sometimes just an artefact of how blood is handled in the lab tube. Moderate counts may cause easy bruising. Very low counts (below 20 × 10⁹/L) increase the risk of spontaneous bleeding including into the skin (petechiae), gums, gut, or rarely the brain. The cause matters enormously — many are benign and easily treated, but some require urgent hospital assessment.

  • What is ITP (immune thrombocytopenia)?

    ITP is the most common cause of isolated low platelets in otherwise well adults. The immune system produces antibodies that destroy platelets. It may be primary (no identifiable cause) or secondary to conditions such as lupus, hepatitis C, HIV, H. pylori infection, or lymphoma. Mild ITP (platelets above 30 × 10⁹/L) without bleeding is often just monitored. When treatment is needed, a short course of corticosteroids is first-line. People with chronic ITP that doesn't respond may be eligible for PBS-listed thrombopoietin receptor agonists (eltrombopag, romiplostim) under specialist care.

  • What is TTP and why is it an emergency?

    Thrombotic thrombocytopenic purpura (TTP) is a rare but life-threatening condition where abnormal blood clotting damages small blood vessels, causing a falling platelet count, breakdown of red blood cells (microangiopathic haemolytic anaemia), and potentially neurological symptoms, kidney injury, and fever. It is caused by deficiency of the enzyme ADAMTS13. Without treatment, mortality approaches 90%. With urgent plasma exchange, survival exceeds 80%. Schistocytes (red cell fragments) on the blood film plus low platelets plus haemolysis demands same-day haematology or emergency assessment. Platelet transfusion is harmful in TTP and should be avoided.

  • How does heparin cause low platelets (HIT)?

    Heparin-induced thrombocytopenia (HIT) is an immune reaction to the blood-thinning medication heparin. Antibodies form against a complex of heparin and a platelet protein, activating platelets and paradoxically causing dangerous blood clots (not just bleeding). Platelet count typically falls 5–10 days after starting heparin. If heparin was given recently and platelets have dropped by 50% or more, HIT should be considered immediately — heparin must be stopped and an alternative anticoagulant started. Avoid platelet transfusions in HIT as they fuel the clotting. A 4T scoring tool (Timing, Thrombocytopenia degree, Thrombosis, oTher cause) guides pre-test probability before ordering antibody testing.

  • When should I go to hospital for a low platelet count?

    Go to an emergency department or call your GP urgently if: your platelet count is below 20 × 10⁹/L; you have active bleeding that isn't stopping (from gums, gut, bladder, or skin); you develop a severe headache or confusion (which could indicate brain bleeding); or if you have fever, jaundice, or your kidneys are not working properly alongside a low platelet count (possible TTP or HUS). If you are pregnant and your platelets are falling, contact your obstetrician. Mild thrombocytopenia above 50 × 10⁹/L without bleeding symptoms can usually be investigated as an outpatient through your GP.
