Carotid stenosis: stroke risk, medical therapy, and when surgery helps
Carotid stenosis is atherosclerotic narrowing of the neck artery supplying the brain. Plaque fragments can break off and cause a stroke or TIA. About 10–15% of ischaemic strokes in Australia involve carotid atherosclerosis.
Management centres on intensive medical therapy — high-dose statin, antiplatelet medication, and blood pressure control below 130/80 mmHg. This is the priority for all patients regardless of stenosis severity.
Carotid endarterectomy benefits symptomatic stenosis of 50% or more, with greatest effect within 14 days of a TIA or stroke. The 2024 CREST-2 trial showed no benefit of surgery over optimised medical therapy for asymptomatic carotid stenosis.
The carotid arteries are the main blood supply to the front of the brain. Atherosclerosis — the same plaque-forming process that narrows coronary arteries — concentrates at the carotid bifurcation in the neck, where the common carotid divides into the internal and external branches. A narrowing there can shed emboli (fragments of plaque or clot) that travel to brain vessels and cause ischaemic stroke or a transient ischaemic attack (TIA). An estimated 10–15% of ischaemic strokes involve large-artery atherosclerosis, and the Stroke Foundation Australia Living Stroke Guidelines are the principal Australian clinical reference for assessment and management.
Two populations need different approaches. Symptomatic carotid stenosis — stenosis of 50% or more on the same side as a recent TIA, stroke, or transient monocular vision loss (amaurosis fugax) within the past six months — carries high early recurrent stroke risk and warrants urgent assessment. Asymptomatic stenosis — found incidentally on imaging without a recent ipsilateral neurological event — carries a much lower annual risk in the modern medical therapy era, and recent evidence has substantially narrowed the indication for surgery.
A. Core clinical — the AU general-practice framework
Recognising carotid stenosis in general practice
Most patients with carotid stenosis present in one of two ways: following an acute neurological event such as a TIA or minor stroke, or as an incidental finding on neck imaging or auscultation during examination for another reason.
Symptoms to recognise — carotid territory:
- TIA — focal neurological deficit resolving within 24 hours (typically minutes to an hour): contralateral hemiparesis or hemisensory loss, aphasia or dysarthria, hemianopia, or transient monocular vision loss in one eye (amaurosis fugax — often described as a curtain coming down).
- Minor stroke — the same deficits persisting beyond 24 hours or confirmed on MRI imaging as an acute infarct.
- Asymptomatic carotid bruit — a murmur heard over the carotid during cardiovascular examination; specific but insensitive (sensitivity approximately 30–50%); its absence does not exclude significant stenosis.
Any TIA or minor stroke is a medical emergency. The Stroke Foundation Living Guidelines recommend urgent assessment, brain imaging, and initiation of antiplatelet therapy plus high-intensity statin within hours, not days.
Assessment in general practice
History:
- Detailed symptom description including timing, resolution, and affected territory.
- Cardiovascular risk factor profile — hypertension, dyslipidaemia, diabetes, smoking, family history.
- Medication history — anticoagulants, antiplatelets, antihypertensives.
- Prior cardiovascular events — myocardial infarction, peripheral arterial disease.
Examination:
- Blood pressure in both arms — asymmetry suggests proximal arterial disease.
- Carotid auscultation — bruit is a useful finding but poor sensitivity; document presence or absence.
- Pulse rhythm — atrial fibrillation as an alternative embolic source requires anticoagulation, not antiplatelet therapy.
- Focused neurological examination — cranial nerves, motor, sensory, coordination.
- Peripheral pulses — evidence of generalised atherosclerosis.
Investigations to arrange:
- Carotid duplex ultrasound — non-invasive first-line imaging; sensitive and specific for moderate to severe stenosis (MBS items 55244/55248).
- ECG — identifies concurrent atrial fibrillation (MBS 11700).
- 24-hour Holter monitor — paroxysmal AF is detected in 5–25% of cryptogenic stroke patients (MBS 11600).
- Echocardiography — assessment for cardiac embolic source (MBS 55135 range).
- Fasting lipid profile, HbA1c, renal function, FBC.
- CT angiography (MBS 56301) or MR angiography (MBS 63335) are arranged for pre-operative planning when revascularisation is being considered.
- Urgent brain CT or MRI for acute stroke or TIA — this is emergency department territory; arrange immediately if in-hours presentation or call 000 out of hours.
Best medical therapy — the cornerstone
The Heart Foundation Australia and Stroke Foundation consistently emphasise that medical therapy is the foundation of carotid stenosis management, regardless of whether surgery is ultimately undertaken. The absolute benefit of revascularisation is added on top of — not instead of — optimised medical management.
High-intensity statin: Atorvastatin 40–80 mg or rosuvastatin 20–40 mg daily. The target LDL is below 1.8 mmol/L per the Heart Foundation for very high cardiovascular risk; the European Society of Cardiology targets below 1.4 mmol/L. Both are PBS-listed. If statin therapy is insufficient, ezetimibe 10 mg daily adds approximately 20–25% further LDL reduction and is PBS-listed. PCSK9 inhibitors (evolocumab, alirocumab) — PBS Authority for selected very high risk patients — can reduce LDL by a further 50–60%.
Antiplatelet therapy: Single antiplatelet (aspirin 100 mg or clopidogrel 75 mg) is standard long-term therapy. After a TIA or minor ischaemic stroke, the POINT trial demonstrated that dual antiplatelet therapy — aspirin plus clopidogrel for 21 days to three months — reduces recurrent stroke by approximately one-third compared with aspirin alone. After three months, dual therapy is stepped down to single antiplatelet to limit bleeding risk. Dual therapy is not indicated for moderate or severe stroke due to haemorrhagic transformation risk.
Blood pressure control: Target systolic blood pressure below 130 mmHg in most patients per eTG Cardiovascular, or below 140 mmHg in frail elderly patients with caution for falls and renal function. First-line combinations include ACE inhibitor or ARB with a long-acting calcium channel blocker or thiazide diuretic.
Glycaemic management: HbA1c target individualised — generally 7% for most patients, 8% for elderly with significant comorbidities. SGLT2 inhibitors and GLP-1 receptor agonists have demonstrated additional cardiovascular event reduction and are preferred agents when clinically appropriate.
Smoking cessation, alcohol moderation, dietary improvement, aerobic exercise: Complete smoking cessation is critical — smoking accelerates plaque progression and substantially increases stroke risk. NHMRC 2020 guidelines recommend alcohol below 10 standard drinks per week. A Mediterranean-style diet — high in vegetables, fruit, legumes, fish, olive oil; limited red and processed meat — supports blood pressure and lipid management. Aerobic exercise of at least 150 minutes per week of moderate intensity is recommended.
Anticoagulation for concurrent AF: When atrial fibrillation is identified alongside carotid stenosis, DOAC anticoagulation (apixaban, rivaroxaban, dabigatran, or edoxaban) is the primary stroke prevention strategy and takes precedence over antiplatelet therapy.
When carotid surgery is considered
The decision to consider carotid endarterectomy requires specialist review by a vascular surgeon. General practice initiates the assessment, arranges urgent carotid duplex, and refers.
- Symptomatic stenosis ≥70% (NASCET): Strong indication for CEA within 14 days of TIA or minor stroke. The North American Symptomatic Carotid Endarterectomy Trial demonstrated an absolute risk reduction of approximately 17% over five years versus medical therapy alone. Delay beyond two weeks substantially erodes the benefit.
- Symptomatic stenosis 50–69%: Modest benefit; individualised based on surgical risk, sex (benefit greater in men), and recency of neurological event.
- Symptomatic stenosis under 50%: No benefit from surgery; best medical therapy alone.

CEA versus carotid artery stenting (CAS): The CREST trial (2010) showed similar long-term efficacy for CEA and CAS, with CEA carrying a higher periprocedural MI rate and CAS carrying a higher periprocedural stroke rate. In patients over 70, CEA is preferred because CAS stroke risk is higher in older patients. CAS is a reasonable alternative for hostile neck anatomy (prior radiation, high surgical risk anatomy) or significant cardiac comorbidity.
B. The evidence base — what the key trials showed
The management of carotid stenosis has one of the most extensively trialled evidence bases in vascular medicine, and the evidence has evolved substantially:
- NASCET (1991) — established CEA for symptomatic stenosis ≥70%. Absolute risk reduction of 17% over five years; benefit concentrated in first two weeks from event. A landmark trial that defined the surgical threshold for three decades.
- ECST (1998) — European trial confirming CEA benefit for severe symptomatic stenosis. Uses a different geometric measurement method, giving values approximately 10–15% higher than NASCET for the same lesion — relevant when interpreting duplex reports using ECST criteria.
- CREST (2010) — compared CEA with CAS in mixed symptomatic and asymptomatic patients. Similar long-term efficacy; periprocedural event profiles differ. CAS was inferior in patients over 70 years due to higher periprocedural stroke risk.
- CREST-2 (2024) — the most consequential recent trial. Enrolled patients with asymptomatic carotid stenosis ≥70% on intensive modern medical therapy and randomised to CEA or CAS versus medical therapy alone. The CREST-2 results found no significant reduction in stroke with CEA or CAS compared with optimised medical therapy. The annual ipsilateral stroke rate in the medical therapy group was less than 1% — far lower than predicted from the 1990s trials conducted before modern statin, dual antiplatelet, and blood pressure regimens. Updated international guidelines are expected to further restrict intervention for asymptomatic disease.
- POINT (2018) and CHANCE (2013) — established dual antiplatelet therapy after minor stroke and TIA, reducing recurrent events by approximately one-third. These have changed the early management of TIA globally, including in Australian practice.
The practical upshot for general practice: intensive medical therapy has become substantially more effective since the 1990s landmark trials were conducted. The incremental benefit of surgery for asymptomatic disease has narrowed to the point where it is no longer broadly recommended. The GP role is to initiate and maintain optimised medical therapy, and to refer symptomatic patients urgently.
C. Secondary prevention in depth — statins, antiplatelets, and blood pressure
Statin therapy is non-negotiable for any patient with established carotid atherosclerosis. The IMPROVE-IT trial demonstrated that adding ezetimibe to statin therapy further reduced major cardiovascular events, with no floor of benefit for LDL reduction identified. If LDL remains above target on maximally tolerated statin plus ezetimibe, PCSK9 inhibitors (evolocumab subcutaneous fortnightly or monthly; alirocumab subcutaneous fortnightly) are PBS Authority-listed for very high risk patients with established atherosclerotic cardiovascular disease.
Antiplatelet choices: Clopidogrel 75 mg daily is PBS-listed as an alternative to aspirin for patients intolerant of aspirin, or as the preferred agent in some clinical contexts. The NPS MedicineWise guidance emphasises that dual antiplatelet therapy should not extend beyond three months without specialist review, given the progressive increase in bleeding risk over time.
Blood pressure medications — the ACE inhibitor or ARB plus long-acting calcium channel blocker combination achieves target in most patients. A thiazide or thiazide-like diuretic is a standard third agent. Resistant hypertension (above 130/80 mmHg on three agents at optimal doses) warrants specialist review.
Lifestyle measures are additive with pharmacotherapy. Salt restriction below 5 g per day, regular aerobic exercise, weight management, smoking cessation, and alcohol moderation each contribute meaningfully to blood pressure reduction and lipid optimisation. Diet: PREDIMED (Mediterranean diet with olive oil and nuts) and DASH trials demonstrate meaningful blood pressure and cardiovascular risk reductions. These align with the Heart Foundation Australia healthy heart diet recommendations and are appropriate to discuss in general practice consultations and GPCCMP allied health referrals.
D. Australian operations
Acute stroke pathway: Suspected TIA or acute stroke warrants immediate 000 call or emergency department attendance. Australia’s comprehensive stroke centre network, coordinated through Stroke Foundation Australia, provides IV thrombolysis (alteplase or tenecteplase) within 4.5 hours and endovascular thrombectomy for large-vessel occlusion within 24 hours for eligible patients. Telestroke services extend this capability to rural and remote areas. Stroke unit admission improves outcomes irrespective of acute reperfusion therapy.
Referral pathway for carotid stenosis: Patients with symptomatic carotid stenosis ≥50% on duplex ultrasound should be referred urgently — within 24–48 hours of a TIA or minor stroke — to a vascular surgery service at a major teaching hospital. Outcomes are captured by the Australian Stroke Clinical Registry (AuSCR). For asymptomatic stenosis, referral is non-urgent; the focus is on medical optimisation in general practice.

Indigenous Australian cardiovascular care: Aboriginal and Torres Strait Islander peoples experience a disproportionate burden of cardiovascular disease, with stroke occurring at younger ages and in the context of multiple comorbidities including hypertension, diabetes, and rheumatic heart disease. Care coordination through community-controlled health organisations (NACCHO affiliates), integration of Indigenous Health Workers, and outreach via telehealth are essential components of secondary prevention in this population.
E. Special populations
Older adults: Periprocedural risk of CEA rises with age and frailty. In octogenarians, patients with significant frailty, or those with limited life expectancy, the modest incremental benefit of surgery — already challenged by CREST-2 for asymptomatic disease — is unlikely to outweigh procedural risk. Intensive medical therapy is the appropriate default. The RACGP Silver Book guides polypharmacy review in elderly patients to optimise adherence and avoid drug interactions.
Women: Carotid atherosclerosis presents on average a decade later in women. Some subgroup analyses suggest the benefit of CEA in symptomatic moderate stenosis (50–69%) may be smaller in women than men. Blood pressure control is particularly important as women have higher stroke incidence relative to their coronary artery disease burden.
Patients with concurrent atrial fibrillation: When AF and carotid stenosis coexist, DOAC anticoagulation is the primary stroke prevention strategy. Antiplatelet therapy is generally not added unless there is another specific indication (for example recent acute coronary syndrome), given the significant increase in bleeding risk with combined therapy.
Younger patients (under 50): Carotid stenosis in younger patients should prompt investigation for non-atherosclerotic causes — fibromuscular dysplasia (string-of-bead appearance on CTA, different from atheromatous plaque), carotid dissection (particularly following trauma, chiropractic manipulation, or spontaneous; characteristic neck pain; anticoagulation or antiplatelet management), vasculitis, or hypercoagulable states (antiphospholipid syndrome, factor V Leiden, protein C or S deficiency).
Diabetes: Diabetic patients have accelerated atherosclerosis and higher stroke rates. Intensive glycaemic management with SGLT2 inhibitors or GLP-1 receptor agonists, alongside statin and blood pressure control, is particularly important. Annual cardiovascular risk assessment and regular carotid review are warranted in patients with diabetes and established carotid disease.
When to escalate
Refer or escalate when:
- Any TIA or suspected acute stroke — call 000 immediately; do not wait for a GP appointment.
- Carotid duplex confirms stenosis ≥50% in the setting of a recent ipsilateral ischaemic event — urgent vascular surgery review, ideally within 24–48 hours.
- Rapidly progressive carotid stenosis on serial surveillance imaging.
- Resistant hypertension or uncontrolled lipid levels despite optimised general practice management.
- Newly identified atrial fibrillation requiring anticoagulation initiation — prompt review.
- Atypical presentation suggesting non-atherosclerotic cause — young patient, possible dissection, inflammatory markers elevated, bilateral involvement.
- Adrenal incidentaloma or other concurrent finding requiring specialist workup.

What is carotid stenosis and why does it matter?
Carotid stenosis is narrowing of the internal carotid artery in the neck, caused by atherosclerotic plaque — the same fatty build-up that narrows coronary arteries. The risk is that a fragment of plaque or a blood clot can break off and lodge in a brain vessel, causing a stroke or TIA. Severity is graded as mild (under 50%), moderate (50–69%), or severe (70% or above). What is a TIA and how urgent is it?
A transient ischaemic attack is a brief episode of focal neurological symptoms — sudden weakness on one side of the body, slurred speech, or vision loss in one eye — that resolves within 24 hours, usually within an hour. A TIA is a medical emergency because stroke risk is highest in the first 48 hours to two weeks. Early assessment, brain imaging, carotid duplex ultrasound, and initiation of antiplatelet plus statin therapy reduce that risk substantially. Any sudden focal neurological symptom, even if brief, warrants immediate emergency department assessment — do not wait.

When is carotid surgery recommended?
Carotid endarterectomy (surgery to remove the plaque) provides a clear benefit for symptomatic stenosis of 70% or more — meaning you have had a TIA or stroke on the same side as the narrowed artery. The benefit is greatest when surgery is performed within 14 days. For moderate symptomatic stenosis (50–69%), the benefit exists but is smaller and depends on individual surgical risk and patient preference. For incidentally discovered asymptomatic carotid stenosis without any recent neurological event, the 2024 CREST-2 trial showed no clear benefit of surgery over well-managed medical therapy.

What long-term medications are needed for carotid stenosis?
Long-term medical therapy is essential regardless of whether surgery is performed. This typically includes a high-intensity statin — atorvastatin or rosuvastatin — targeting LDL below 1.8 mmol/L; antiplatelet medication such as aspirin or clopidogrel; and blood pressure medicines targeting under 130/80 mmHg. After a TIA or minor stroke, short-term dual antiplatelet therapy (aspirin plus clopidogrel) for 21 days to three months significantly reduces recurrent stroke. If atrial fibrillation is also present, a DOAC anticoagulant is the primary stroke prevention strategy.

Do I need carotid artery screening?
Routine population screening for carotid stenosis in people without symptoms is not recommended. The annual stroke risk from asymptomatic carotid stenosis is low in the modern medical therapy era, and the 2024 CREST-2 data show surgery adds little over good medical treatment. Your GP may arrange a carotid duplex ultrasound if you have had a TIA or stroke, if an abnormal sound (bruit) is heard over the artery on examination, or as part of investigation for unexplained neurological symptoms. For most people, the most effective strategy is cardiovascular risk factor management.
